Celiac, Wheat Allergy, or Gluten Sensitivity? They're Not the Same Thing
Celiac disease is an autoimmune condition where gluten triggers damage to the small intestine. A wheat allergy is an immune (IgE-mediated) reaction to wheat proteins specifically, and doesn't necessarily involve barley or rye. Non-celiac gluten sensitivity causes real symptoms after eating gluten but without the autoimmune markers of celiac or the antibody response of a true allergy. All three call for avoiding gluten or wheat, but how strict that avoidance needs to be is genuinely different for each.
Celiac disease: autoimmune, and the strictest of the three
When someone with celiac eats gluten, their immune system attacks the lining of the small intestine: real, measurable damage over time, even without obvious symptoms in the moment. Diagnosed through blood tests followed typically by an endoscopic biopsy. Because the damage can happen silently, avoidance needs to be strict and lifelong. This is where the FDA's gluten-free labeling threshold of under 20 parts per million matters most.
Wheat allergy: immune, but a different pathway
IgE-mediated, like a peanut or milk allergy. Importantly, a wheat allergy is about wheat specifically, not gluten broadly: someone with a wheat allergy may be able to tolerate barley or rye, a meaningfully different restriction than celiac requires.
If gluten seems to be the problem, testing for celiac disease should generally happen before going gluten-free, because going gluten-free first can make celiac testing inaccurate later.
Non-celiac gluten sensitivity: real symptoms, no antibody trail
Bloating, fatigue, brain fog, digestive discomfort after gluten, without testing positive for celiac's markers or a wheat allergy's IgE response. Currently a diagnosis of exclusion, meaning celiac and wheat allergy generally need to be ruled out first. The FDA's "gluten-free" labeling rule matters enormously to someone with celiac disease; for someone with NCGS, the threshold that actually bothers them may be looser, since their reaction isn't governed by the same biological mechanism. Neither is wrong. They're just not the same problem.
